Transitions to Adulthood for young people in Middlesbrough needing ongoing support from Adult Social Care.
In Middlesbrough we know how important Preparing for Adulthood is for young people and the people who support them, we recognise the anxieties this period in someone’s life can bring.
Transitions for young people with disabilities are supported by a multi-agency Transitions Forum, held quarterly, where the current worker, and other professionals who know the young person well, work hard to identify a clear pathway into adulthood for them.
This process is supported by a multi- agency 6 monthly Transition Tracking group which ensures we continue to work to achieve the best possible outcomes for each person.
When the appropriate Adult Social Care support is identified there may be a period of joint working and assessment prior to the young person’s 18th birthday. During this time the adult worker will work alongside the children’s worker.
If the individual is eligible for support the adult worker will become the Allocated Worker from the age of 18 years.
Young people who do not have a diagnosis of a disability but have eligible needs as an adult (Care Act 2014) can be referred up to 6 months prior to their 18th birthday via Adult Social Care Access team or be referred, where there are safeguarding concerns, via Middlesbrough’s VEMT process.
We aim to ensure that young people preparing for adulthood, regardless of their additional needs, will have equality in relation to their transition to adult social care, and where an individual is likely to be eligible for ongoing social care support as an adult, they have one planned transition at the age of 18 years.
