Short Break Service for children with disabilities and their families.

The CWD Short Break Service sits within the Children with Disabilities Service and supports children/young people with a disability, who meet the eligibility criteria for CWD support (see Children with Disabilities Team Eligibility Criteria for more details). The CWD Short Break Service can work with children and young people aged 0-17, who meet the Moderate, Severe or Profound categories of the Threshold of Need.


The service offers early intervention and a route to accessing positive community activities and support, via an Early Help Assessment (Short Breaks Assessment). This is Tier 2 intervention and not the same level as Stronger Families. This means that not all families who have a child with a disability require an in-depth social work assessment to receive the support that they need. Each support package will be personalised to the child/young persons interests and needs, as well as those of the family. If there is a need for overnight respite, paid for by Social Care, then ongoing social work involvement is required.

If funding is agreed by the CWD Resource Panel, the Short Break Facilitator works with the family to get the support up and running. The Short Break Facilitator will be the first point of contact for the family and they will be keeping in regular contact to ensure things are going smoothly and get ready for any reviews that are required.
