Preparation for Adulthood Leaver Preference Form

From Yr 9 onwards consideration needs to be given to how young people prepare for adulthood.

The preparation for adulthood form has been developed to capture information which the SEND Code of Practice say must be considered for every young person from Yr 9 onwards. 

This form will be completed annually for young people and will form part of their annual EHCP review.

The preference form has been developed to support planning for post 16 provision and to help with securing placements for young people as early as possible.
