Short Break Service
Short Break Service for children with disabilities and their families.
The Middlesbrough Short Break Service is available in order to support families to access short break provision in the local area. A Social Worker will work alongside one of our Short Break Facilitators in order to complete a focussed, light touch short break assessment to determine what support is required for your family and child. If throughout the process of completing the short break assessment, it becomes clear to the social worker that further support or involvement is needed, this can easily be transitioned into an Assessment of Need and full access to social work support can be provided.
Once this assessment has been completed, the Social Worker will present the request to a panel and if agreed, this is then handed over to a designated Short Break Facilitator to work with the family to get the support up and running. The Short Break Facilitator will be the first point of contact for the family and they will be keeping in regular contact to ensure things are going smoothly and get ready for any reviews that are required.
