Transitions Team

Middlesbrough's Transitions Team provide support to young people with disabilities from the age of 14 years until the age of 25. They support the young person when their overall need links to their transition through adolesence to adulthood and where they are likely to require services, advice and/ or support due to the impact of their disability.

The young person will have a medium to high level of support needs and will be eligible for support from social care as a child or adult. They may have:

  • A learning disability or
  • Autism
  • Have a physical disability, which may include a sensory support issue or
  • Have other complex needs

The young person may not be able to do many of the things that other young people of their age are able to do and may need a higher level of supervision.

A young persons transition can include such things as:

  • Leaving school
  • Going to college
  • Getting a job
  • Changing where you live
  • Organising social and leisure activities
  • Relationships  

Every young person's transition will be different but the process starts in Year 9.  Young people who are aged 13-14 are supported to start thinking about their future.  

There are a few ways a young person can be referred to the Transitions Team, they are as follow:

  • If the young person already has social work involvement then their current social worker will attend a meeting called Transitions Forum to discuss information about the young person.  This makes sure everyone works together and helps us to get things right to make sure the young person has a smooth transition.
  • The young person will have a Year 9 annual review which will begin to look at transitions and a transitions plan and may also include their Education, Health and Care plan.  The review is arranged by an officer from education and will take place at school.  At this point it may be felt that the young person would benefit from social work input and education can make a referral to the Transitions Team.
  • Parents/ carers & professionals can make a referral on behalf of the young person.
